Home Private Duty Care is administered by Registered Nurses or Certified Nursing Assistants. In accordance with the client's Plan of Care recommended by the physician or case manager, nurses are responsible for performing tasks necessary to maintain continuity of care.

These services would benefit the following clients:

• Have recently been discharged from a Health Care Facility
• Have undergone major surgery and are in need for safe and speedy recovery assistance
• Require rehabilitative therapy under the supervision of a home health care professional
• Are increasingly becoming more dependent on others for their activities of daily living due to illness or old age
